Job Description

Purpose of Role

To provide our Home Care clients with a range of daily care services in their own home. As well as to demonstrate a commitment to best practice in aged care while maintaining an optimal level of dignity, comfort and quality of life for our clients. As well as to demonstrate a commitment to best practice in aged care while maintaining an optimal level of dignity, comfort and quality of life for our clients.

 

The Personal Care Assistant will report changes in a resident’s health status to the Registered Nurse or Care Partner.

 

Key Responsibilities

  • Provide a client centred service that identifies appropriate communication strategies to meet the complex needs of clients and build strong relationships with clients within professional boundaries.
  • Provide agreed assistance with care and services using the guidelines provided under Certificate III in Aged Care
  • Establish appropriate relationship with the client to assess and analyse their strengths, needs, abilities and resources
  • Ensure standards of care within the national framework for comprehensive community care assessment and continuous improvement
  • Ensure the design and facilitation of care and services which may include, but is not limited to, bathing (showering and personal hygiene), toileting (continence management), skin care, nasal care, grooming, medication, transferring/ mobility, therapy, assistance with eating, socialisation, meal preparation and domestic services
  • Effectively manages the care of clients according to the documented care plans.
  • Prioritise workload and respond effectively to changing situations
  • Uses health care resources effectively and efficiently for optimal care
  • Uses appropriate strategies to promote self-esteem, dignity, integrity, safety, security and comfort to clients
  • Collaborate with the health care team to ensure policy and guidelines are effective

Desired Skills and Experience

Qualifications & Training

 

  • Cert III in Aged Care, Individual Supports (Aged Care) or Cert III AIN
  • Medication Competency (desired)
  • First Aid and CPR Certificate (HLTAID011)

Requirement 

 

  • Own reliable vehicle

Experience

 

  • Experience working in a similar role, preferably in the aged care or health industry
