Job Description

 

Purpose of Role

The purpose of this role is to deliver high-quality, person-centred nursing care to aged care clients in their homes, supporting their physical, emotional, and mental wellbeing. The role ensures safe, culturally appropriate, and evidence-informed care aligned with individualised home care plans and the Aged Care Quality Standards. It involves clinical decision-making, coordination of services, mentoring of care staff, and collaboration with multidisciplinary teams to promote optimal health outcomes and dignity for clients living in the community.

 

Key Responsibilities

  • Ensure the effective delivery of high-quality, timely, and culturally appropriate care to clients in their homes, supporting individuals with physical or mental health challenges, in alignment with individualised home care plans.
  • Demonstrate a commitment to client safety and continuous improvement in the quality of care delivered, using evidence-informed practice. Support the development, implementation, and evaluation of person-centred care plans that meet the Aged Care Quality Standards.
  • Monitor the health and functional status of clients and maintain clear, regular communication with the Care Manager regarding changes in condition, care needs, or responses to care interventions.
  • Provide advanced clinical decision-making, in-home nursing interventions, and coordinate care services while offering leadership and support to care staff working in the field.
  • Lead and mentor personal care workers, promoting safe, effective care practices and building their capacity to provide holistic, client-centred support in the home environment.
  • Establish and maintain therapeutic and professional relationships with clients, families, and other healthcare professionals to ensure the delivery of comprehensive and effective home care.
  • Assess care complexity in a home care setting and delegate tasks appropriately to ensure client safety and efficiency, following HPS protocols and procedures.
  • Prioritise and organise home visit schedules to ensure timely and responsive care, adapting to clients' changing needs and emerging health concerns.
  • Provide compassionate emotional support to clients and their families, recognising the importance of comfort, reassurance, and dignity in home-based care.
  • Administer medications safely in line with the 5 Rights of Medication Administration and document accurately in accordance with legislative and organisational requirements.
  • Engage in quality improvement activities including audits, service development, and best practice reviews aimed at improving client outcomes in the home care setting.
  • Collect and report accurate data related to the client’s health, care outcomes, and service effectiveness; contribute to care planning and review processes.

 

Provision and Coordination of Care

  • Conduct thorough, evidence-based nursing assessments in the client's home setting.
  • Interpret assessment data accurately to identify client needs, risks, and preferences.
  • Develop and document home care plans in consultation with the client, carers, and health professionals, ensuring continuity of care and client engagement.
  • Utilise home care resources effectively and efficiently to deliver safe, high-quality services.
  • Evaluate client progress toward identified care goals and adjust care plans as required.

 

Collaborative and Therapeutic Practice

  • Build and maintain respectful therapeutic relationships with clients and families to foster trust and empowerment in the care relationship.
  • Apply strategies that uphold client dignity, safety, independence, and informed decision-making.
  • Collaborate with GPs, allied health professionals, and external services to ensure seamless care delivery across disciplines.
  • Support the development and implementation of home care policies and clinical guidelines through collaborative practice with the healthcare team.

Desired Skills and Experience

Qualifications & Training

 

  • Current registration as a Registered Nurse with AHPRA
  • A commitment to maintaining Continuing Professional Development (CPD)
  • Annual CPR accreditation

Experience

 

  • Experience in community nursing, home care, or similar aged care roles
  • Previous experience supervising or supporting care workers or enrolled nurses

Skills & Competencies

 

  • Solid understanding of evidence-informed practice and the ability to apply critical thinking in autonomous settings
  • Strong time management and organisational skills to manage multiple home visits and adapt to client needs
  • Capability to work independently and exercise professional judgement while remaining accountable for nursing decisions
  • Sound knowledge of the responsibilities and notification obligations of Registered Health Practitioners
  • Familiarity with the Australian Privacy Act 1988 and principles of confidentiality in home care settings
  • Empathetic and compassionate approach to aged clients living in the community
  • Excellent interpersonal, verbal, and written communication skills to collaborate with multidisciplinary teams, clients, and families
  • Proficient in electronic health systems and digital care planning tools
  • Knowledge of aged care legislation, funding, and care documentation requirements (Desirable)
